False PHD is a passive item introduced in The Binding of Isaac: Repentance that fundamentally alters how pills affect your character. It acts as the dark mirror to the standard PHD item. While it makes almost all pills negative or neutral, it compensates by turning your bad pill habits into a powerful offensive engine. Core Mechanics

Pill Identification: Like the standard PHD, it reveals the name and effect of all pills before you consume them.

Pill Transformation: It automatically forces any potential positive pills to invert into their negative versions.

Retroactive Damage Buffs: Every time you swallow a stat-decreasing pill (e.g., Tears Down, Luck Down, Speed Down), you receive a permanent +0.6 Damage Up. This effect also applies retroactively to any bad pills consumed prior to picking up the item.

Black Heart Generation: Consuming non-stat-reducing harmful or annoying pills (e.g., Amnesia, Addicted!, R U A Wizard?) rewards you by spawning a Black Heart on the floor. Key Strategic Synergies

PHD: If you pick up both the authentic PHD and the False PHD, they form a unique combination where pills can be either good or bad again. Crucially, any bad pills you take will still trigger the False PHD damage upgrades and Black Heart spawns.

The Wafer: Lowers the damage penalty of a Bad Trip pill down to just half a heart while still spawning a full Black Heart, resulting in a net health gain.

Placebo: Allows you to repeatedly trigger a specific stat-down pill to rapidly stack infinite damage multipliers, or reuse an immunity-frame window to generate endless Black Hearts.
